Persons wishing to enroll in the University of Texas - Houston Health
Science Center Postgraduate School of Dentistry are requested to submit
the following documents. Your application will be processed when all the
required documents have been received:

A completed application form and a letter of
application

A $10.00 non-refundable application fee and a recent
2" x 2" photograph

Official transcripts from all colleges and
universities attended including dental program must be on file one
year prior to enrollment in the Postgraduate School of Dentistry
(Transcripts are on file if you have attended any UT - HHSC school.
Check with the Office of the Registrar.)

Copy of Dental National Board Examination Scores
(Scores are on file if you are a graduate of the UT - HHSC Dental
Branch DDS program. Check with the Office of the Registrar.)

Dean's letter of recommendation (Letter should be
forwarded directly to the Office of the Registrar and must include
cumulative grade point average and class standing if these are not
shown on the official transcript. Letter is not required for UT -
HHSC Dental Branch DDS graduates.)

Three letters of recommendation in addition to
Dean's letter are required. (Letters of recommendation from PASS are
acceptable for applicants who are going through PASS. Applicants who
graduated more than two years prior to this application may submit
letters of reference from fellow practitioners in the community or
from their supervisors if employed by any government service.)

Satisfactory scores obtained on the Graduate Record
Examination (GRE) (The GRE is required for all applicants who
wish to pursue a Graduate (M.S.) program in Endodontics and
Periodontics and for applicants who wish to pursue Postgraduate
(Certificate) programs in Orthodontics and Prosthodontics. General
information and testing dates can be obtained by writing the Office
of the Registrar.)

Applicants who are nationals of countries where
English is not the parent tongue are requested to submit scores from
TEST OF ENGLISH AS FOREIGN LANGUAGE (TOEFL). (General information
and testing dates can be obtained by writing the Office of the
Registrar.)

A PASS application is required for General Dentistry
(AEGD), General Practice Residency (GPR), and Pediatric Dentistry.
(Application to PASS is optional for UT - HHSC Dental Branch fourth
year DDS students who are applying only for AEGD or GPR.)
Personal Interviews may be arranged at either the
request of the applicant or the institution.
The application deadline for Oral Surgery is
September 1 of the year before expected enrollment. NOTE: The Oral
Surgery program has its own application that must be completed instead
of this application. Request an Oral Surgery Application packet if
applying.
The deadline for application to the Endodontics,
Orthodontics, Periodontics, and Prosthodontics program is October 1 of
the year before expected enrollment.
The deadline for applications to the General
Dentistry (AEGD), General Practice Residency (GPR), Oral Pathology and
Pediatric Dentistry programs is November 1 of the year before expected
enrollment.
The deadline for application to the Dental Public
Health program is March 15 of the same year.
